Most women from Sonipat diagnosed with ovarian cancer are already at Stage III or Stage IV — this is because the ovaries are deep inside the pelvis and early disease causes no clear symptoms. Stage III–IV ovarian cancer is serious, but it responds to treatment and can be managed for years with the right care.
What Advanced Ovarian Cancer Means
- Stage III — cancer has spread within the pelvis and abdomen, involving peritoneal surfaces, omentum, or lymph nodes
- Stage IV — spread to the liver parenchyma, lungs, or pleural fluid
Despite sounding alarming, Stage IIIC ovarian cancer has median survival of 3–5 years with modern treatment — and some patients achieve long-term remission.
Cytoreductive Surgery — The Foundation of Treatment
The goal of surgery in advanced ovarian cancer is to remove all visible tumour — “optimal debulking” — leaving behind less than 1 cm of residual disease. This is technically demanding, requiring removal of tumour from the bowel surface, diaphragm, spleen, and liver. Dr. Monisha performs these extended procedures in coordination with general surgery colleagues at Fortis.
HIPEC — Heated Chemotherapy at Surgery
For selected patients achieving complete cytoreduction, HIPEC (Hyperthermic Intraperitoneal Chemotherapy — heated chemotherapy washed through the abdomen during the operation) reduces the risk of peritoneal recurrence. Maintenance Therapy
After platinum-based chemotherapy, BRCA-mutated patients benefit from PARP inhibitor maintenance therapy (olaparib, niraparib), extending progression-free survival. Dr. Monisha’s team tests every suitable patient for BRCA mutation and recommends targeted maintenance when applicable.
